When two avatars are present in a Warband, does your opponent get a fate for killing either of them after the Effigy Power is called, or just the one declared as your avatar?

If you have two avatars in your Warband, only one is your chosen avatar, and only that one provides a Fate if killed after the Effigy Power is called as per the Honour rule.

How does Divine Intervention interact with damage applied by combat manoeuvres?

If a friendly model to the hero with Divine Intervention is reduced to 0 HP as a result of a Basic Attack, Divine Intervention would trigger during Step 5 of the Attack Process to bring them back to 1 HP. The attack process would then continue to Step 6 Combat Manoeuvres. If damage is applied in Step 6, the model would be reduced to 0 HP for a second time and Divine Intervention cannot trigger again, resulting in the model's death.

Does a model with Fly take damage from Treacherous Ground?

A model with Fly ignores the damage from Treacherous Ground only on a Charge action and only if the model does not end on a Treacherous Ground hex.

How does a level 3 Thorgar with Enraged Assault in play interact with the Elixir of Life, Isabel's Divine Intervention or similar abilities?

When Thorgar is reduced to 0 HP, the Elixir of Life, Divine Intervention or similar abilities will trigger first healing him for 1 health.
